Pomaika‘i Wound Care is seeking a compassionate and skilled Nurse Practitioner with advanced training in wound management to join our mobile wound care team across Hawai‘i. In this role, you’ll provide high-quality care for patients with acute and chronic wounds in a variety of settings, including homes, long-term care facilities, and skilled nursing environments. You’ll work closely with our medical director, collaborate with interdisciplinary teams, and play a vital role in helping patients heal and improve their quality of life.
Perform thorough wound evaluations (size, depth, tissue condition, drainage, infection risk).
Develop and implement personalized treatment plans using evidence-based approaches (dressings, debridement, negative pressure wound therapy, etc.).
Provide education and support to patients and families on wound prevention and home management.
Document care clearly and accurately using EMR and wound photography.
Partner with physicians, nurses, therapists, and care coordinators to ensure continuity of care.
Contribute to clinical quality initiatives and help refine best practices in wound management.
Required:
Current Hawai‘i APRN license (FNP, AGNP, or ANP).
Active national NP certification.
Wound care credential (CWCN, CWOCN, WCC, or equivalent).
Minimum 1 year of nursing experience (wound care or specialty care preferred).
BLS certification (ACLS a plus).
Strong communication, critical thinking, and documentation skills.
Preferred:
CWCN or CWOCN advanced certification.
Experience with hyperbaric oxygen therapy.
EMR/wound documentation software experience.
Bilingual skills (English + Hawaiian or Pacific Islander languages).
